CROSS has four phases that are implemented in six months of programming. During each phase intensive case management is provided with an emphasis on gang involvement, family structure, skills development, and pro-social behaviors.
Phases
Phase 1: Orientation-3 weeks
In phase one an overview of the program is discussed with the participant and their family. Coordination of services is established with all professionals involved. Contributing factors are identified for the participant and their family. Assessments are administered to gather perceptions of gangs and pro-social behaviors. Case managers are meeting with the participant weekly throughout the four phases.
Phase 2: Implementation-12 weeks
Individual meetings with a CROSS Coordinator are implemented twice a week. Sessions include case management, motivational interviewing, change talktraining and skill building. Participants are actively involved in a weekly Cognitive Behavioral Treatment group for 1 hour, for 12 weeks. times a week. The participant and the family also are involved in individual/family therapy one time a week.
Phase 3: Application-6 weeks
Weekly communication is conducted with case manager, participant, and the family to ensure skills are being implemented and needs are addressed. Individual/family therapy is still ongoing through this phase. Coordination of community resources are reviewed and monitored.
Phase 4: Preservation -3 weeks
Participant and the family are evaluated and are able to maintain change and navigate resources in the community. Individualized service plan goals have been met and implemented in the home, school, and community. An after care plan is created to include participants support systems, crisis resolution, and maintenance. The post assessments are administered to gather perceptions of gangs and pro-social behaviors.
