Probation officers and JJS case managers please fill out a completed referral form. Attach a copy of all court orders and the PRA/PSRA, as these are required before services can be provided.
Once a referral is received coordinators will ensure the participant meets criteria and make contact with the probation officer or JJS case manager to inform them of the participant's acceptance or denial. Contact will also be made with the family to schedule an intake appointment to begin services. Probation officers and JJS case managers are welcomed to participate in the intake meeting, but it is not necessary.
